Fill out the form below to apply for membership.

Before starting, please be sure to have all the necessary documents on hand:

  • List of your 40 program visits with location and contact information

  • Your three references with contact information - please do not use other members of your group practice or anyone on the TCA Membership Committee as a reference

  • Scans of your degree(s) and credential(s)

  • Contract if dual relationship exists

  • Resume and headshot

Unfortunately this form is not able to save incomplete applications.

Upon submitting your application, you will be redirected to pay our $60 application fee. Our Membership Committee will review your application as soon as possible. Once they have reviewed your application, they will reach out with further information.
